Under-Eye Shadows and Tear Trough Treatment in Marylebone, London
Under-eye shadows and tear trough treatment in Marylebone, London softens the hollow between the lower eyelid and the cheek so the eyes look rested and brighter. Helena assesses whether hollowing, pigment or skin quality is the real cause before recommending filler, polynucleotides or Sunekos.
Overview
Understanding under-eye shadows & tear troughs
Tear trough shadows are the dark hollows that run diagonally from the inner corner of the eye towards the cheek, the area that makes people look tired even after a good night's sleep. The shadow forms when the fat and bone beneath the lower eyelid recede with age, leaving a groove that catches the light from above. Some people have it from their twenties because of their bone structure.
Not every dark circle is a tear trough. Sometimes the darkness is pigment in the skin itself, sometimes it is thin, crepey skin showing the blood vessels beneath, and often it is a mix. Each needs a different approach, which is why Helena examines the area in different lighting and asks you to look up, down and to the side before suggesting anything.
Where true hollowing is the cause, a small amount of hyaluronic acid dermal filler placed deep along the trough can smooth the transition between eyelid and cheek, with minimal downtime. Where skin quality is the issue, polynucleotides or Sunekos work on the skin itself, improving thickness, hydration and tone over a course. Not everyone is suitable for filler in this area, and Helena will say so honestly rather than treat a problem filler cannot fix. Results vary between individuals.
Why it happens
What causes under-eye shadows & tear troughs?
- Age-related loss of fat and bone support beneath the lower eyelid
- Inherited facial structure, with a deep-set trough present from a young age
- Thinning of the delicate under-eye skin, revealing blood vessels beneath
- Pigmentation in the under-eye skin, more common in darker skin tones and with rubbing or allergies
- Poor sleep, dehydration and screen fatigue, which make existing shadows more visible
- Significant weight loss, which reduces the fat pad that supports the area
Treatment approach
How does Helena treat under-eye shadows and tear troughs in Marylebone?
Dermal Fillers
From £300For true hollowing, Helena places a small amount of soft hyaluronic acid filler deep along the trough to smooth the step between eyelid and cheek. This is a delicate area that demands careful assessment, and she will only treat it where she is confident it will help. Minimal downtime. From £300.
See treatment detail →Polynucleotides
From £350Polynucleotides injected into the under-eye skin support hydration, elasticity and repair, so thin, crepey or dull skin gradually looks smoother and brighter. A course of 3 to 6 sessions, 4 to 6 weeks apart. From £350.

Common
questions
Am I suitable for tear trough filler?
Not everyone is, and Helena will tell you honestly. Filler works best when the shadow is caused by a true hollow, the skin is thick enough and there is little puffiness. If the darkness is mainly pigment, or the under-eye area is puffy or the skin thin, filler may not help and she will suggest a different route.
Is there downtime after tear trough treatment?
Minimal, for most people. You may see mild swelling or a small bruise under the eye for a few days, which concealer usually covers. Helena asks you to sleep slightly propped up the first night and to avoid rubbing the area, alcohol and heavy exercise for 24 hours. Most clients return to their day straight afterwards.
How long do results last?
Tear trough filler often lasts longer than filler elsewhere, commonly 12 months or more, because the area moves less. Polynucleotides and Sunekos work on the skin itself and build over a course, with maintenance roughly every 6 to 9 months to hold the improvement. Helena reviews you after treatment and plans maintenance around how your skin responds.
Why do my dark circles look worse some days?
Because shadows depend on light and fluid. Poor sleep, salty food, alcohol and allergies all cause mild puffiness or dilate the blood vessels under thin skin, so the same trough looks darker. Overhead lighting deepens it too. That is why Helena assesses you in more than one light and asks about your sleep and habits before recommending treatment.
What are the risks of tear trough filler?
The under-eye area is delicate, so it carries more risk than most areas and demands a cautious approach. Possible issues include swelling, bruising, visible lumps or a bluish tint if filler sits too close to the surface, and rarely more serious vascular complications. Helena uses small amounts, places filler deep and discusses every risk with you before treatment.
